Steps for Students Who Will Do an Internship

  • FORM-1: Internship Obligation Form: This form will only be filled out for companies that require proof that the internship is mandatory. In other cases, it is not necessary to fill out this form. After printing and filling it out, the form should be taken to the Department Secretary to obtain approval from the Department Chair (stamp/signature).
  • FORM-2: Application and Acceptance Form for the Workplace: (Important, first step in internship paperwork). This form is required for students to be insured against Work Accidents and Occupational Diseases. The top part of FORM-2 will be filled out by the student and approved by the department internship coordinator (before taking it to the company). Then the student will take this form to the company where the internship will take place, and the bottom part will be filled in and signed/stamped by the company. This form, along with FORM-3 or FORM-4 (whichever applies), must be submitted to the department internship coordinator at least 20 days before the internship start date. Otherwise, it will be too late for insurance procedures, and no insurance will be provided for the internship.
    Note: This form will not be accepted unless it is signed by the department coordinator before being taken to the company.

    Important: Some companies provide an internship acceptance letter on their own letterhead that includes the information from FORM-2 and is signed and stamped. This acceptance letter can be used instead of FORM-2. In that case, FORM-2 is not needed, and the company's letter can be submitted with FORM-3 or FORM-4 (no additional signatures required) to the internship coordinator at least 20 days before the internship begins in order to start the insurance process.
  • Insurance Document: After all procedures related to FORM-2 and (FORM-3 or FORM-4) are completed and submitted to the Internship Coordinator, the INSURANCE ENTRY DOCUMENT will be obtained by students a few days before the internship start date via the E-Government portal. (The company-issued internship acceptance letter mentioned above is also valid in place of FORM-2).
  • FORM-3: Declaration and Commitment for those receiving healthcare through family: This document should be filled out only by those who receive healthcare through their family and submitted to the internship coordinator along with FORM-2.
  • FORM-4: Declaration and Commitment for those not receiving healthcare through family: This document should be filled out only by those who do not receive any healthcare through their family and submitted to the internship coordinator along with FORM-2.
  • FORM-5: Internship Evaluation Form: The student section (upper part) of this form must be filled out and a photo must be attached (can be added digitally and then printed). This form must first be approved by the department internship coordinator and the department chair, and then submitted to the company on the internship start date. There is no time restriction for FORM-5 until the internship begins. FORM-5 procedures can be carried out in parallel with FORM-2 or separately up to the start of the internship. Similarly, this form will be invalid if it is not signed by the department coordinator before being submitted to the company.
  • FORM-6: This form will only be filled out for companies that will pay a wage to the intern and request 30% of this wage from the government. (If the company does not request it, there is no need to submit this form.) Also, this form should never be signed by the department internship coordinator.
  • FORM-7: Performance Assessment Form within ABET scope: This form does not need to be signed by the department coordinator; it should simply be completed with the student’s information and submitted to the company along with FORM-5. At the end of the internship, the company will fill out both FORM-5 and FORM-7, place them in a sealed envelope, and either give it to the student or mail it to the department.
  • NOTE: After the internship ends, students must submit their internship report, their transcript covering the last semester, and the sealed envelope obtained from the company to the internship coordinator within the first 15 days of the following academic term.
